Pandemic Hairdressing Form (Pad of 50 | A4 size)

In line with Government guidelines, this form has been developed to help hair salons and hairdressers safely welcome customers back into their premises.

This pad of 50 not only captures the customer's name, stylist's name and date, it also asks each visitor to confirm:

• I knowingly and willingly consent to having hair and salon service(s) during the COVID - 19 pandemic.
• To prevent the spread of contagious viruses and to help protect each other understand that I have to follow the salons strict guidelines.
• I understand that due to the frequency of visits of other clients, the characteristics of the virus and the characteristics of hair services, that i have increased the risk of contracting the virus by merely being in the salon.
• I understand the COVID-19 virus has a long incubation period, during which carriers of the virus may not show symptoms and still be highly contagious.
• It is impossible to determine who has it and who does not given the current limits in virus testing.
• I confirm that I have not travelled outside the UK by commercial airline, boat, bus or train in the past 14 days.
• I agree not to come to the salon with the following symptoms of COVID-19
• Fever, High temperature, shortness of breath, loss of sense of taste or smell, dry cough, runny nose, sore throat.
• I understand, read and completed this form truthfully. I agree that this constitutes full disclosure and that it supersedes any previous verbal or written disclosures. I understand that this document is to provide the best possible guest experience when visiting this salon.
