Lifting Equipment Permit To Work Self Duplicating Forms Pad of 50

These popular Lifting Equipment permit pads come as triplicate sets, glued at the head with a board back. This format means these are stored easily and simple to use, making them ideal for busy sites that need to issue permits regularly. 

Each pad has 50 Permits, made up of an original top copy and two duplicate sheets. 

These permits are designed to ensure Contractors comply with all relevant Health and Safety Legislation and the Company's health policy. They also help businesses establish whether a contractor has Public and Employee Liability Insurance, and can provide a Method Statement on request.

How it works
Prior to starting any relevant work this self-duplicating RISK ASSESSMENT should be carried out by a contractor. On completion the top copy is held by the contractor and the secondary copy retained as a permanent record by the office.

Starting with the fundamental question "Are you qualified / trained to undertake this work?" and ending with the sign off of responsibility, amongst others, this permit to work poses the following questions:

• Has the type of load been fully assessed?
• Do you have a certificate of competence to undertake this work?
• Has the lifting equipment been thoroughly examined by a competent person to ensure it is safe to operate, and that it has adequate strength and stability for the proposed use?
• Are subsequent periodic examinations in place to make sure it remains safe?
• Has the risk of Proximity Hazards been assessed and made as low as is reasonably practicable?
