SSP’s Accident, Incident and Illness report book hold 50 individually numbered sheets consisting of a top copy and a self duplicating copy. Ideal for schools and colleges, this report form is designed to be completed as soon as any significant incident or accident occurs.

To comply with the Health and Safety act 1974, post incident, the responsible person must notify the enforcing authority after any incident takes place.

With tear-out pages, this is also compliant with GDPR data protection legislation.

Each form covers:
Name, location, the type of injury sustained, detail of treatment administered, and what happened to the pupil next (eg, parents contacted, remained at school, picked up)

Features and Benefits
By using the report form you will be complying with the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) Act 1995 and also with the Data Protection Act 1998. This is particularly important if the injured party decides to take the organisation to a court.
This form is self duplicating and comes with an extra 1 sheets, this means that all the required people (including the injured party) has access to this form, instead of sharing just one copy.
Finally by reporting the incident at work and the company taking remedial action, it may stop anything similar or perhaps more serious happening again.
