Personalised SSP School Accident Incident and Illness Report self duplicating book

This book can be fully personalised by adding your organisation's name, logo and questions you wish to ask. Your only limit is the size of the book, although on request we can quote to increase the size to A4.

Information on the format of this book

Each form typically covers:
Name, location, the type of injury sustained, detail of treatment administered, and what happened to the pupil next (eg, parents contacted, remained at school, picked up). However you can change this in anyway you see fit.

This form is self duplicating and comes with an extra 1 sheets, this means that all the required people (including the injured party) has access to this form, instead of sharing just one copy.

Keeping you compliant and safe

To comply with the Health and Safety act 1974, post incident, the responsible person must notify the enforcing authority after any incident takes place.

By using the report form you will be complying with the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) Act 1995 and also with the Data Protection Act 1998. This is particularly important if the injured party decides to take the organisation to a court.

With tear-out pages, this is also compliant with GDPR data protection legislation when handled in line with your GDPR procedures.

Finally by reporting the incident at work and the company taking remedial action, it may stop anything similar or perhaps more serious happening again.
